A valid DMCA notice should include:

1. Your physical or electronic signature, as the owner of an exclusive right that is allegedly infringed, or a person authorized to act on the owner's behalf.
2. Identification of the copyrighted work claimed to have been infringed, or a representative list if multiple works are covered by a single notice.
3. Identification of the material claimed to be infringing, with information reasonably sufficient to let us locate it, such as the page URL.
4. Information reasonably sufficient to let us contact you, such as an address, telephone number, and email address.
5. A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
6. A statement that the information in the notice is accurate, and under penalty of perjury that you are authorized to act on behalf of the owner of the exclusive right.

If material you posted was removed and you believe the removal was a mistake or misidentification, you may file a counter-notification with [email protected]. A valid counter-notification should include:

1. Your physical or electronic signature.
2. Identification of the material that was removed and the location where it appeared before removal.
3. A statement under penalty of perjury that you have a good-faith belief the material was removed as a result of mistake or misidentification.
4. Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the appropriate court and will accept service of process from the complaining party.

mathcheck may, in appropriate circumstances, restrict or remove access for users or contributors who are found to be repeat infringers.

Misrepresenting that material is infringing, or that it was removed by mistake, may expose the sender to liability for damages. Submit notices only when you have a genuine, good-faith belief.
